How often does it happen, and what does it affect?

A stressful process is worth noticing. For systems routing, this assessment also looks for recurrence and a practical business consequence.

Where do you look to know what happens next?

This section looks for a usable source of truth and the handoff between a signal and its next action.

Keep it practical. This assessment uses your readiness and desired outcome to avoid recommending more than you need.
